    By submitting this Membership Application, I authorize Banner House at T Bar M to request and review information necessary to assess my qualifications for membership. This may include, but is not limited to, law enforcement records and credit reports reflecting my credit history. I agree to hold the Club harmless from any actions taken as part of this review process. Additionally, I authorize relevant individuals and entities to provide any requested information to the Club and release them from any liability in doing so. I understand that the Club’s Membership Committee, appointed by Club Management, will evaluate each prospective Member based on established criteria, which may include credit and background checks, reference checks, and other assessments as deemed necessary by the Club. However, Banner House does not discriminate based on race, color, religion, sex, ancestry, national origin, age, disability, medical condition, sexual orientation, gender identity, or marital status. Applicants may be required to complete a full Membership application, provide letters of reference if requested, and participate in interviews as part of the review process. All membership applications, agreements, interview notes, and deliberations of the Membership Committee will remain strictly confidential and accessible only to Club Management. There is no set timeline for application review or verification of membership qualifications, as this process is conducted at the sole discretion of the Club.
